Heat or Ice? A Simple Guide to Getting It Right

The most common question we get asked, and the honest answer is less rigid than most advice suggests. Here is a practical rule for choosing, and the safety points that matter more than the choice itself.

Almost every patient asks this within the first ten minutes, usually expecting a firm rule. The honest answer is that the difference between them is smaller than most advice implies, and that neither one is a treatment. Both are pain-relief tools that make it easier to do the thing that actually drives recovery, which is moving.

That said, there is a sensible way to choose.

That last row is not a cop-out. There is no good evidence that choosing "wrong" causes harm, and the comfort itself has value, it helps you sleep and keeps you moving.

What ice actually does

Ice reduces pain. That is its reliable effect, and it is a useful one.

What it does not clearly do is speed healing. The old assumption that suppressing inflammation improves recovery has weakened considerably, inflammation is an active, necessary part of tissue repair, and blunting it aggressively may work against you. This is why modern acute-injury guidance de-emphasises ice compared with the old RICE protocol, and why compression and early gentle loading have moved to the front.

So use ice for comfort in the early days if it helps. Do not use it in the belief that you are accelerating repair, and do not use it to numb pain so you can push through something you should not be doing.

What heat actually does

Heat increases local blood flow and makes muscle and connective tissue more pliable. It is genuinely useful for:

  • Muscle spasm and guarding, very common in acute lower back pain
  • Morning stiffness from arthritis
  • Warming up a stiff joint before exercises, which is why we often suggest a hot shower before a home programme
  • Long-standing tightness with no acute swelling

The most practical use of heat is ten minutes before your exercises. It makes the session more comfortable, and the exercises are the part that changes anything.

Doing it safely

The choice matters less than the application. Almost all harm from either comes from technique:

  1. Never apply ice directly to skin. Wrap it in a damp cloth. Ice burns are entirely avoidable and surprisingly common
  2. 15 to 20 minutes maximum, then a break of at least an hour
  3. Check the skin partway through, especially with heat
  4. Never sleep on a heat pack or with an electric blanket on one area: a low-grade burn over hours is a real risk
  5. Heat should be comfortably warm, not hot

When to avoid them entirely

Skip both, or check first, if you have:

  • Reduced sensation in the area, you cannot feel a burn developing. This matters particularly for people with diabetic neuropathy
  • Poor circulation or peripheral vascular disease
  • Broken skin, an open wound, or an active infection
  • An area with a deep vein thrombosis, suspected or confirmed
  • Cold hypersensitivity conditions such as Raynaud's, for ice specifically

Avoid heat over an acutely swollen, hot, red joint. That combination can indicate infection or an inflammatory flare and needs assessing rather than warming.

The part that matters more

It is worth saying plainly: neither heat nor ice will resolve a musculoskeletal problem. They make you comfortable enough to move, sleep and load the tissue, and that loading is what produces change.

If you find yourself relying on one of them daily for weeks, that is the signal to get the underlying problem assessed rather than to buy a better heat pack.
